Microscope-assisted endodontics in Market Harborough

Root canals can be narrower than a human hair, curve sharply and hide behind calcified dentine. An operating microscope gives the clinician bright, magnified sight of the inside of the tooth — the difference between guessing and seeing. Every root canal treatment and retreatment at our practice is performed under one.

What the microscope changes

Under normal vision the floor of a molar's pulp chamber is a dark space a few millimetres across. At 8 to 20 times magnification, with light aimed straight down the tooth, the same space reveals the openings of each canal, the faint lines that mark a hidden second canal, cracks in the dentine and the edges of old filling material.

  • Finding every canal. Upper molars very often have a fourth canal that is missed without magnification; missing it is one of the most common causes of failure.
  • Conserving tooth structure. Because the clinician can see, the access can be kept small and dentine preserved — which keeps the tooth stronger in the long run.
  • Managing complications. Separated instruments, blocked canals, perforations and calcified canals can be assessed and often dealt with rather than abandoned.
  • Diagnosing cracks. Fine cracks can be seen and their extent judged, which matters when deciding whether a tooth is worth treating.

The other tools that work with it

Magnification is the foundation, but it is combined with other techniques during treatment:

  • Electronic length measurement determines precisely where each root ends, so canals are cleaned and filled to the correct length.
  • Ultrasonic instruments remove posts and old materials, uncover canal openings and activate the disinfecting solutions.
  • Flexible nickel-titanium files follow curved canals while preserving their natural shape.
  • Activated irrigation drives the disinfectant into the side branches and fins of the canal system that files cannot reach.
  • Digital radiography keeps X-ray doses low and lets us check progress during the appointment.

What it means for you

You will not notice the microscope much — it sits above you while you lie back, and the rubber dam does most of the work of keeping you comfortable. What you benefit from is a more complete clean of the canal system, a smaller access cavity and fewer surprises. Treatment under magnification does take a little longer, which is why our appointments are scheduled generously rather than squeezed between other patients.

Judgement first, technology second

Equipment does not make decisions; it makes good decisions easier to carry out. Diagnosis, planning and an honest view of a tooth's prognosis come first. If you have been told a tooth is "too difficult" or "not worth treating", a consultation under the microscope is a reasonable way to find out whether that is really the case — see our page on saving your natural tooth.

Root canal retreatment

A second chance for a tooth whose earlier root canal treatment has failed: the old filling is removed, missed or infected canals are found and treated, and the tooth is sealed again.
